The Story Behind the Jabodebek LRT Accident

The Indonesian National Transportation Safety Committee (KNKT) released the investigation results of the Jabodebek light rail transit (LRT) collision on October 25, 2021. KNKT chairman Soerjanto Tjahjono said the investigation results found an operator error. The train set (TS) 29 technicians got distracted due to using cell phones while working. "The technicians did not focus on monitoring the train speed and position," Soerjanto said on December 21, 2021.
NTSC findings in the Jabodebek LRT accident:
- The analog speedometer needle on the MC1 TS 29 stops at 50 kilometers per hour.
- The standard operating procedure (SOP) for shunting states that the train speed should be immediately lowered to 3 kilometers per hour when seeing a train ahead.
- The shunting SOP has not set the method of communication. In that incident, the technician used the Whatsapp app from a cell phone.
- The point of the shunt stop is not clear.
- In the curve before the crash site, the technician's view was blocked by trees.
- The second download of HMI TS 29 does not match the incident's date and time.
- SDiag (onboard diagnostics) has not been configured.
- TS 29 technician was distracted due to cell phone use.
- TS 29 technician did not focus on monitoring the train speed and position.
- The sun visor is partially covered, thus limiting the technician's free view.
- Cabin ergonomics are not optimal (rotating seat design).
- There is a cover plate on the emergency brake button.
